It’s Lacrosse Time!
 
My name is Dr. Jonathan W. Silva and I am a dentist and a lacrosse coach.   I have coached high school lacrosse for over 10 years. Currently, I coach the St. Louis Samurai Club Lacrosse Team and am the defensive coordinator for the Eureka Varsity Team.
 
Not only as a dentist and coach, but as a former high school and college lacrosse player, I understand the importance of wearing a mouth guard while playing sports.  Protecting a player’s safety with a proper mouth guard is important to reduce sports related dental injuries and minimize concussions.  These mouth guards must be worn properly, not chewed on, during practice and games to be effective. 
 
A custom fit mouth guard will stay in the players’ mouths better than a “boil and fit” stock mouth guard.  The improved fit of a custom mouth guard will also allow for improved communication with teammates during games (very important for good team defense).
 
My dental team and I would like to provide, at no cost, a custom made vacuum formed mouth guard to the players of Eureka Lacrosse.  Please NOTE that if you are currently in orthodontic braces, we are unable to do impressions for a mouth guard due to the constant changes in your dentition during orthodontic treatment.
 
To make the custom mouth guards, your son will need to make an appointment for an impression of his upper teeth at our office.  Please call our practice Monday through Thursday at 636-391-9904 to schedule this visit.  I'm at this office on Monday, Wednesday, and Thursday.  Consent forms will need to be signed and brought to the impression appointment or filled out in our waiting room.  The consent form, which must be filled out by all players under the age of 18 is attached here:
